COMPLEX NEUROIMMUNE PRESENTATIONS
PANS PANDAS & complex neuroimmune presentations
Looking beyond the diagnosis to understand a child’s individual immune, infectious, inflammatory and neurological pattern.
WHEN SOMETHING HAS CHANGED
A careful place to begin
Some families arrive with an established PANS or PANDAS diagnosis. Others simply know that their child has changed and no one has been able to explain why.
Not every child has a textbook presentation, a positive infection test or a single clear trigger. The purpose of an consultation is not to force symptoms into a label; it is to understand what changed, when it changed, what is happening now and which factors may be clinically relevant.
A SHORT EXPLANATION
What are PANS and PANDAS?
PANS is a clinical term used for a sudden onset of severe obsessive-compulsive symptoms or markedly restricted eating together with other significant behavioural, emotional, neurological or physical changes.
PANDAS is the term used when a similar pattern is associated with a recent group A streptococcal infection.
Complex by nature
No individual laboratory test confirms PANS or PANDAS. A thorough evaluation considers the symptom timeline and excludes other possible explanations.
SIGNS WORTH DISCUSSING
What may warrant a closer look
These experiences can have many causes. A child does not need to show every sign, and having one of these symptoms does not establish PANS PANDAS.
Sudden behavioural change
Severe anxiety, rage, marked mood change, new separation anxiety or unusually intense emotional instability.
OCD, tics or movement changes
Sudden OCD, motor or vocal tics, repetitive behaviours, unusual movements or changes in handwriting.
Eating, sleep or toileting changes
Food refusal, restricted eating, sleep disturbance, frequent urination or new bedwetting.
Regression or reduced function
Loss of previously acquired skills, school decline, sensory intolerance, cognitive change or withdrawal.
THE WIDER CLINICAL PICTURE
Consultation is directed by the child’s history
Testing is never a fixed package. If it is useful, it is selected around the clinical question, prior results and the family’s budget.
Timeline & history
Illnesses, exposures, medication changes, flares, developmental history, what improved and what caused a reaction.
Infection & immune context
Relevant infections, immune response, inflammation and existing specialist or medical findings.
Gut, nutrition & tolerance
Digestive symptoms, food acceptance, nutrient status, elimination and the feasibility of any changes.
Environment & recovery
Sleep, stressors, water damage or mould history where relevant, and the child’s capacity to tolerate intervention.
HOW WE WORK
Measured, phased and individualised
The aim is not the longest protocol. It is a practical plan with a clear purpose for each next step.
- 01
Understand
Review the full history, current priorities, records and relevant results.
- 02
Prioritise
Decide what genuinely needs attention first and which investigations may change the plan.
- 03
Introduce carefully
Where appropriate, introduce recommendations in a logical sequence so responses can be monitored.
- 04
Review & adapt
Use follow-up and focused support to respond to new information, tolerance and progress.
PRACTICAL FOUNDATIONS
Support should fit the child and the family
For children with sensory sensitivities or restricted eating, an immediately restrictive plan can add stress rather than help. Recommendations are adjusted around tolerance, priorities and what is realistically manageable.
Families receive clear explanations and symptom-tracking guidance. Focused WhatsApp support can be used for agreed questions or reactions between appointments.
WHY QUALITY MATTERS
A lower price does not always mean better value
A supplement may be poor value if its ingredients, dose, form or manufacturing quality are not suitable for the individual. Brand, sourcing, formulation, testing and clinical relevance matter alongside budget.
Independent analyses of selected supplement categories have reported discrepancies between labels and tested contents. These findings do not mean that every supplement is poor quality; they underline why product selection deserves care.
WHEN MORE SUPPORT IS NEEDED
Advanced options are never automatic
For some complex cases, further medical consultation or specialist collaboration may be appropriate. Any prescription, infusion-based or advanced biological treatment must be considered by appropriately qualified medical clinicians and in the context of the individual patient.
YOU DO NOT NEED TO ARRIVE WITH A DIAGNOSIS
The first step is to make sense of the story.
Whether you have a diagnosis, suspect an infectious or inflammatory trigger, or simply know that something changed, an initial consultation can help identify the most relevant priorities and decide what is worth exploring.
